If you have only one, title it Appendix. If you have more than one, they will be titled Appendix A, Appendix B, Appendix C, and so on. You decide which is Appendix A or B or C by the order in which they are referred to in your paper.
Appendices go after the References section, and are titled Appendix A , Appendix B etc. You might be able to use numbers instead if you wish, but I only see published reports with lettered appendices.
